The information model depicts the documentation processes of patient care by nurses and/or other allied health professionals in any healthcare setting using the CCC System (version 2.5)’s free coded standardized nursing terminologies. The CCC System information model illustrates the flow of nursing knowledge, represented by coded concepts of the nursing process (assessment, diagnoses, goals, plan/implementation interventions, actual outcomes) as interactive, interrelated and with continuous feedback. It also illustrates not only the information relationships between the nursing diagnoses and their outcomes and the nursing interventions and their action types, but also between the six steps/standards of the nursing process. The model arrows are bi-directional, indicating the continuous flow and feedback among the three major concepts: nursing diagnoses, nursing interventions and nursing outcomes. It provides the base for documenting nursing practice and POCs using the CCC terminologies, which are described in detail on our framework page.
